Once our team receives your completed form, we’ll send you a secure email containing the required information your organization needs to complete to join the program. Completing the “Get Started” form will also allow us to keep you up to speed with important updates about the program.

On-boarding kits must be completed and submitted by April 5th for a July 1st effective date of coverage. Submissions received after April 5th will have a coverage effective date of August 1st or later.
Submissions received thereafter will require a 60 day implementation period with a 1st of the month coverage effective date.
On-boarding completion date of April 18th (missed deadline) = August 1st coverage effective date.
On-boarding completion date of May 18th = August 1st coverage effective date.
On-boarding completion date of June 18th = September 1st coverage effective date.
